How to protect your testicles in a sauna
Short answer: sit lower on the bench, keep sessions shorter, and cool the area directly. Of those three, direct local cooling is the only one that lets you keep the session you actually came for. The reason any of it matters is that the tissue in question is supposed to run cooler than the rest of you, and a sauna removes that gap.
Why does sauna heat matter for testicles specifically?
Because sperm production is one of the few processes in the human body that requires a temperature below core. The scrotum sits outside the body for exactly that reason. In fertile men under normal conditions, scrotal temperature measures roughly 33 to 35 degrees Celsius, against a core temperature of about 37. That gap of two to four degrees is not incidental. It is the operating condition.
Roger Mieusset, the French andrologist who has spent a career measuring this, presented a summary to an ICNIRP and WHO thermal workshop in 2015 with two numbers worth holding onto. A scrotal temperature above 35 degrees Celsius is a risk factor for sperm output. At 36 degrees, measured depression in count, motility and morphology follows.
A Finnish style sauna runs at 80 to 100 degrees Celsius. You do not need a model to see the problem.
How much of a temperature rise does it actually take?
Less than most people assume, and this is the finding that reframes the whole question.
In 1967 Robinson and Roch insulated the scrotums of ten men for fifteen hours a day. The measured temperature rise was 0.8 degrees Celsius. Not eight degrees, not three. Under one. Sperm output fell roughly 50 percent between weeks three and nine, and roughly 80 percent by week ten.
A larger epidemiological cohort, Hjollund and colleagues in 2002, found a dose response in ordinary men going about ordinary lives: sperm concentration fell about 40 percent for every one degree Celsius increase in median daytime scrotal temperature.
So the problem is not that a sauna is extreme. The problem is that spermatogenesis is unusually sensitive to sustained elevation, and a sauna is a very efficient way to produce one.
What the sauna research actually measured
The cleanest study on the sauna specifically is Garolla and colleagues, 2013, in Human Reproduction. Ten healthy men with normal semen parameters took two sessions per week for three months, fifteen minutes each at 80 to 90 degrees Celsius. That is an ordinary session at an ordinary frequency.
At the end of three months, sperm count and motility were both strongly impaired at a significance level of P less than 0.001. Upstream markers moved too: normal histone to protamine substitution fell from 78.7 percent to 69.0, chromatin condensation from 70.7 percent to 63.6, and mitochondrial function from 76.8 percent to 54.0. Sex hormones did not change significantly, which tells you the effect is local and thermal rather than endocrine.
Six months after the sessions stopped, every measure was back to baseline. It is ten men, so treat it as a strong signal rather than a settled number, but it is the best measured thing we have.
What actually works, in order
1. Sit lower. A sauna is strongly stratified. The air at the top bench can be twenty degrees or more above the air at the bottom bench in the same room. Moving down one level is free and it is the single largest change most people can make. It also makes the session less pleasant, which is why almost nobody does it.
2. Shorten the session. Exposure is cumulative. Fifteen minutes twice a week was enough to produce the Garolla result, so time is doing real work here.
3. Go less often. Effective, and the option people are least willing to take, because frequency is where most of the cardiovascular benefit in the Finnish population research comes from. Nobody who has read that literature wants to tell you to use the sauna less.
4. Cool the area directly. This is the only option on the list that changes the variable you care about without changing the session. It is also the one people improvise badly.
5. Do not sit next to the heater. Radiant heat falls off with distance and the bench directly beside a stove is a meaningfully different environment from the far end of the same bench.
What people improvise, and why it fails
Go through any sauna forum thread on this and the same handful of workarounds come up. A bag of ice on the lap. A cup of ice water. A cold wet towel. Underwear made for post vasectomy recovery, which comes with ice packs and was designed for a man lying on a sofa.
They fail for the same reason each time. Nothing holds the cold in position. Ice in a bag slides, leaks, and ends up on the bench. A wet towel in 90 degree air is dry in minutes. The vasectomy underwear works, briefly, because it was never designed to sit in a hot room. The physics is right and the delivery is wrong.
Insulating pouches with no cold source are a separate mistake. Insulation slows heat transfer in both directions. In a room that is 50 degrees hotter than your skin, slowing the transfer is worth something, but it does not remove heat, and nothing that does not remove heat is going to hold you under 35 degrees Celsius for a twenty minute session.

Common questions
Is it dangerous to put something frozen against your skin in a sauna?
The gel pack sits inside a fabric pocket, not against bare skin. Direct skin contact with anything at freezer temperature is uncomfortable and can cause a cold burn, which is why every sensible design puts a layer in between.
Do I need to do anything if I sauna once a week?
Probably not. The Garolla protocol was twice weekly for three months and the effect tracks with cumulative exposure. Occasional use is not what was tested and there is no good evidence it does anything lasting.
How long does it take to recover?
A full cycle of sperm production runs about 74 days, so recovery is measured in months rather than days. In the 2013 study, measures were still depressed at three months after cessation and back to baseline at six.
Does cooling during the session actually change the outcome?
Nobody has run that trial. Heat exposure demonstrably impairs the parameters, and reducing local heat is the mechanically obvious lever, but the specific study of sauna with cooling against sauna without cooling does not exist. We would rather say that plainly than overstate it.
What about a towel folded under you?
It helps, in the sense that the bench itself is hot and conducted heat is real. It does nothing about the air, which is the larger term.
Should I stop using the sauna if we are trying to conceive?
That is a question for your doctor and it is worth actually asking rather than reading a blog about it. Bring the Garolla paper with you.
